Volunteer Forms

Our number one priority is safety. All of our deans, faculty, jr. faculty and other volunteers need to complete the following forms prior to their arrival at camp. If you are age 21 or older and are not a resident of Indiana or Michigan, please contact the camp office to get the Child Protective Services form for your state.

NEW FORM! All volunteers/staff need to complete the Personnel Record Form. All previous forms have been combined into this one form. Please complete this questionnaire. If you are over the age of 21, please look below to also complete the Central Registry Clearance.

NEW PROCESS!!  All Indiana Residents age 21 or over need to complete their own Central Registry Clearance by clicking the link below and completing this form.  You will need to scan and email them to Kelly at kelly@michianacamp.org or fax them to 269-683-4402.
